POLICIES

• Dr. Carlo's practice does not accept insurance. Payment is due on each scheduled appointment in the form of all major credit cards, personal checks, or cash.
• Patients will be provided with a receipt from the doctor for each visit which can be submitted to your personal medical insurance plan for reimbursement.
• Please provide at least 48 hours notification (848-468-1307) if an appointment needs cancellation or rescheduling. Failure to do so will lead to fees equal to the amount of the service charge for the scheduled appointment.
• Upon arrival for an appointment, please be seated in the waiting area. The doctor will greet you in this area when available.
